ONSITE BIOMETRIC HEALTH SCREENINGS
Your Leprino Foods primary work location will offer onsite biometric health screenings in September and October of 2024. Appointment scheduling will be made available in the 3rd quarter of 2024. WHAT IS A BIOMETRIC HEALTH SCREENING
A biometric health screening is a short examination that checks risk indicators for certain health conditions. The health screening provides you with an explanation of what your health risk numbers mean and, if indicated, where you should take action to optimize your health and wellbeing.
WHAT TO EXPECT AT YOUR HEALTH SCREENING
Your screening appointment will last approximately 30 minutes. HealthYou clinicians will measure 15 different health risk indicators which are collected through a finger-stick blood draw, a lifestyle habits survey, a weight and waist circumference measurement, and a blood pressure reading. A HealthYou clinician will review your results with you during your screening.
HOW TO PREPARE FOR YOUR HEALTH SCREENING
You are not required to fast prior to your screening; however, for most accurate results you may choose to not eat or drink anything other than plain water 8-hours prior to the health screening. Plain water is highly encouraged for hydration and increased blood flow. In addition to drinking plenty of water, be sure to take any medications as prescribed and wear comfortable clothing. This will allow HealthYou clinicians to easily measure waist circumference on your skin and not over your clothing.
BIOMETRIC SCREENING RESULTS AND ACTIVITY POINTS INFORMATION
A copy of your individual biometric health screening results will be made available to you immediately after you complete your onsite screening. On a monthly basis, your points will be reported to Anthem. Your Leprino Foods sponsored biometric health screening results are accessible electronically via the Anthem website: https://anthem.com and through the Sydney App. Updated point information should be available to you by the second week of the month following your screening date or within 30 days, whichever comes first.
NOTICE REGARDING YOUR BIOMETRIC HEALTH SCREENING
Your collected biometric health screening information will be coded so your identity (name, Social Security Number, date of birth, or other unique identifiers) cannot be known to outside parties. This is known as de-identification. Some of this de-identified information, including aggregate reports of overall population health data, may be shared with outside parties for the purpose of evaluating and planning various employee health and wellness activities and programs.
By participating in the biometric health screening, you are giving permission for the collection of your personal health information by the HealthYou staff AND permission to disclose ONLY anonymous, coded, de-identified data for review by outside parties. You also agree to the purpose of collecting this health information and understand the strict confidentiality of your individual results. Your results will not be shared with your employer.
